There is no waiting period. You can enroll in the SIP at any time. If you have not enrolled in the Plan within 30 days from your date of hire, you will be automatically enrolled in the Plan at a contribution rate of 3% of your pretax eligible earnings. Your contributions will be invested in a BlackRock LifePath® Index Portfolio fund, based on your date of birth, which is the default fund for the SIP. However, we encourage you to take an active role in the Plan and to choose a contribution rate and investment options that are appropriate for you. You should also designate your beneficiaries when you enroll in the Plan.

If you do not wish to contribute to the SIP, you must change your contribution rate to 0% within the first 30 calendar days of your employment. You may change your contribution rate at any time.

Through automatic payroll deduction, you can contribute from 1% to 75% of your eligible pay on a pretax basis, up to the annual IRS dollar limits. If you are under age 50 the IRS contribution limit is $19,000 for 2019. If you will be at least age 50 during the year, your plan may allow you to defer an additional $6,000 as catch-up contributions.

Cleveland Clinic will match 50% of each pretax dollar you contribute on the first 6% of pay that you defer to the Savings and Investment Plan.

You have access to a range of investment choices in your plan, including conservative, moderately conservative, and aggressive funds. We encourage you to take an active role in your plan and choose investment options that best suit your goals, time horizon, and risk tolerance. You can review available investments in your Plan Enrollment Guide available in the Tools & Resources section above, under the Forms & Documents tab. If you do not select specific investment options in the plan, your contributions will be invested in a BlackRock LifePath® Index Portfolio fund, based on your date of birth, which is the default fund for the SIP.

The BlackRock LifePath® Index Portfolios are designed for investors expecting to retire around the year indicated in each portfolio's name. The portfolios are managed to gradually become more conservative over time as it approaches the target date. The investment risk of each BlackRock LifePath® Index Portfolio changes over time as its asset allocation changes. The portfolios are subject to the volatility of the financial markets, including that of equity and fixed income investments in the U.S. and abroad, and may be subject to risks associated with investing in high-yield, small-cap, and foreign securities. Principal invested is not guaranteed at any time, including at or after the target dates.

You are always 100% vested in your contributions to the Cleveland Clinic SIP, as well as any earnings on them. Cleveland Clinic's matching contributions and any earnings become vested after you have completed three years of service.

Beneficiaries - shows details. Click to open.

Your beneficiary or beneficiaries will inherit your account in the event of your death. You should consider identifying a beneficiary when you enroll in your plan, and updating the information if you experience a life-changing event such as a marriage, divorce, birth of a child, or death in the family.

Fidelity's Online Beneficiaries Service, available through NetBenefits® offers a straightforward, convenient process that takes just minutes.

Go to 'Profile' in the navigation bar at the top of your NetBenefits® page and click on the 'Summary tab' and then 'Beneficiaries'.

You are generally allowed to withdraw money from your plan when you leave your employer, retire, or become permanently disabled. Also you may be eligible for a distribution if you have a financial hardship as defined by your plan. Withdrawals may be subject to income taxes and, if they occur prior to you becoming age 59½, a 10% early withdrawal tax penalty.

Note: The taxable portion of your withdrawal that is eligible for rollover into an individual retirement account (IRA) or another employer's retirement plan is subject to 20% mandatory federal income tax withholding, unless it is directly rolled over to an IRA or another employer plan. (You may owe more or less when you file your income taxes.) Be sure you understand the tax consequences and your plan's rules for distributions before you initiate a distribution. You may want to consult your tax advisor about your situation.
