The Statewide Hybrid Plan covers firefighters and police officers from departments that complete the re-affiliation process with FPPA and elect coverage under this plan. The re-affiliation process was first established by Colorado State Statute and became effective January 1, 2004. This Plan may also cover full-time clerical staff or other personnel employed by an affiliated fire protection district, fire authority or a county improvement district whose services are auxiliary to fire protection. If an employer covers its members under the Statewide Hybrid Plan, participation begins as of the date of hire assuming contributions are properly remitted to FPPA. If a member selects this plan via the re-affiliation process participation begins upon the effective date of the re-affiliation assuming contributions are properly remitted to FPPA.
What is the employee contribution rate? Employees must currently contribute 8% of their base salary as a mandatory employee contribution.* These contributions will be made on a pretax basis and are contributed to the plan through automatic payroll deduction. This percentage is set by the FPPA Board and can be increased.
Employer contributions* Participants currently receive 8% of their base salary as employer contributions. The employer contribution percentage is set by the FPPA Board and can be increased.
Voluntary employee contributions You may also elect to make voluntary employee contributions to the plan. Voluntary contributions are made on an after-tax basis and contributed to the plan through automatic payroll deduction. Contact FPPA for additional information.
* Upon request by the employer, the FPPA Board will permit higher employer or mandatory employee contributions. Sixty-five percent of members in the department must approve the higher contribution rate. Total contributions made on behalf of a participant for a plan year shall not exceed the lesser of the annual addition limit under code section 415(c), or 100% of compensation.
The Plan also offers Fidelity® Personalized Planning & Advice, a managed account service that lets you delegate the day-to-day management of your workplace savings plan account to professional investment managers. Fidelity's experienced professionals evaluate the investment options available in your plan and identify a model portfolio of investments appropriate for an investor like you. The service then invests your account to align with this model portfolio and provides ongoing management of your account to address changes in the markets, your plan's investment lineup, and changes in your personal or financial situation. With a managed account, you can take advantage of Fidelity's resources and experience to help ensure that:

You are always 100% vested to any contributions you make. You will vest to your employer contributions according to the following schedule:

Participants will also become 100% vested upon death, disability, or the attainment of normal retirement age (55).
Your beneficiary or beneficiaries will inherit your account in the event of your death. You should consider identifying a beneficiary when you enroll in your plan, and updating the information if you experience a life-changing event such as a marriage, divorce, birth of a child, or death in the family.
Fidelity's Online Beneficiaries Service, available through NetBenefits® offers a straightforward, convenient process that takes just minutes.
Go to 'Profile' in the navigation bar at the top of your NetBenefits® page and click on the 'Summary tab' and then 'Beneficiaries'.
If you do not have access to the Internet or prefer to complete your beneficiary information by paper form, please call 1-800-343-0860. Please note, beneficiary designations made with Fidelity are only valid for your FPPA accounts with Fidelity. You must contact FPPA directly to change your beneficiary for any of your other FPPA-related benefits.
Withdrawals from the Statewide Hybrid Plan, Money Purchase Component are allowed under any of the following conditions:
Members are not required to withdraw funds upon separation of service, unless the member is over age 70½. Loans and hardship withdrawals are not permitted. If you are under age 55, the taxable portion of your withdrawal may also be subject to a 10% early withdrawal penalty, unless you qualify for an exception to this rule. The plan document and current tax laws and regulations will govern in case of a discrepancy.
Members in the Statewide Hybrid Plan, Money Purchase Component may also convert all or a portion of their account balance to a monthly lifetime benefit. The conversion must be done in one lump sum and is a one-time irrevocable decision. This monthly lifetime benefit is eligible for benefit adjustments and the member may also choose to provide for survivor benefits.
