You are immediately eligible to participate in the Plan if you are in a regularly scheduled permanent position, so long as you are at least 18 years old.

To enroll, simply click the Enroll Now button above and follow the prompts. Your enrollment becomes effective once you indicate how much you'd like to contribute, also known as the deferral. You will generally begin to see your contributions deducted from your paycheck in the next pay period, or as soon as administratively possible.

If you have not enrolled in the Plan within 30 days of your hire date, you will be automatically enrolled at a contribution rate of 5% of your pretax eligible earnings. Based on your date of birth and assuming a retirement age of 65, you will be invested in a Vanguard Target Retirement investment, with a corresponding target retirement date. We encourage you to take an active role in the Plan and choose a contribution rate and investment options that are appropriate to you. If you do not wish to contribute to the Plan, you must change your contribution rate to 0% within the first 30 calendar days of your employment.

Through automatic payroll contributions, you can contribute between 0% and 70% of your eligible pay, up to the annual IRS dollar limits.

The contribution limit for 2019 is:
- $19,000 for employees age 49 and younger
- $25,000 for employees who will be at least age 50 or older during the year

Annual additions to your Plan account -- that is, contributions made by both you and Salem Health -- may not exceed 100% of your pay or $56,000 for 2019 (whichever is less).

You have access to a range of investment choices in your plan, including conservative, moderately conservative, and aggressive funds. We encourage you to take an active role in your plan and choose investment options that best suit your goals, time horizon, and risk tolerance. Default Investment Option: If you do not select specific investment options in the plan, your contributions will be invested in a Vanguard Target Retirement Trust II Series investment.

Each investment offers a blend of stocks, bonds and short-term investments in a single investmenet. Vanguard professionals manage the investments based on a target retirement date. The mix of investments, or asset allocation, adjusts periodically to be more aggressive in the beginning and become more conservative over time. Of course, as with any investment, Vanguard Target Retirement Trust II Series investments involve risk and may lose money at any time, even near or after your retirement.

When you are automatically invested in a Vanguard Target Retirement Trust II Series investment, we select the target date closest to the year you might retire, based on your date of birth and assuming a retirement age of 65, at the direction of your employer or plan sponsor. If we do not have a valid date of birth on file at Fidelity, your contributions may be invested in the Vanguard Target Retirement Income Trust II investment. Vanguard Target Retirement Trust II Series investments are designed for investors expecting to retire around the year indicated in each fund's name. Except for the Vanguard Target Retirement Income Trust II investment, the investments' asset allocation strategy becomes increasingly conservative as it approaches the target date and beyond. Ultimately, they are expected to merge with the Vanguard Target Retirement Income Trust II investment. The investment risks of each Vanguard Target Retirement Trust II Series investment change over time as its asset allocation changes. They are subject to the volatility of the financial markets, including equity and fixed income investments in the U.S. and abroad and may be subject to risks associated with investing in high yield, small cap and, commodity-related, foreign securities. Principal invested is not guaranteed at any time, including at or after their target dates.

The Plan also offers Fidelity® Personalized Planning & Advice, a managed account service that lets you delegate the day-to-day management of your workplace savings plan account to professional investment managers.

Fidelity's experienced professionals evaluate the investment options available in your plan and identify a model portfolio of investments appropriate for an investor like you. The service then invests your account to align with this model portfolio and provides ongoing management of your account to address changes in the markets, your plan's investment lineup, and changes in your personal or financial situation.

With a managed account, you can take advantage of Fidelity's resources and experience to help ensure that:

  • Your investments are managed through the ups and downs of the market.
  • You're keeping your accounts aligned with your goals through annual reviews and check-ins.
  • Your account is actively managed to create an opportunity for long-term gains while managing the risk associated with investing.

When you are "vested" in your savings, it effectively means the money is yours to keep. You are always 100% vested in all contributions you make to the plan, plus any Matching contributions made by Salem Health, as well as any earnings on them.

Beneficiaries - shows details. Click to open.

Your beneficiary or beneficiaries will inherit your account in the event of your death. You should consider identifying a beneficiary when you enroll in your plan, and updating the information if you experience a life-changing event such as a marriage, divorce, birth of a child, or death in the family. In the event of your death, if you don't identify a beneficiary with Fidelity, your account may not be paid out according to your wishes.

Fidelity's Online Beneficiaries Service, available through NetBenefits® offers a straightforward, convenient process that takes just minutes.

Go to 'Profile' in the navigation bar at the top of your NetBenefits® page and click on the 'Summary tab' and then 'Beneficiaries'.

Withdrawals - shows details. Click to open.

You are generally allowed to withdraw money from your plan when you leave your employer, retire, reach age 59½ and have completed 5 years of eligible service, or become permanently disabled. Also you may be eligible for a distribution if you have a financial hardship as defined by your plan. Withdrawals may be subject to income taxes and, if they occur prior to you becoming age 59½, a 10% early withdrawal tax penalty.

If you separate employment and your account balance is $5,000 or more, you can leave your monies in the Plan. If your account balance is between $1,000 and $4,999, your account will be rolled over to an individual retirement account (IRA) with Fidelity. If your balance is less than $1,000 when you separate employment, it will automatically be distributed to you. Although your plan account is intended for the future, you may borrow from your account. Plan rules allow you to borrow up to 50% of your employee contribution account balance (i.e. Salem Health contributions are not available as loan proceeds).

The minimum loan amount is $1,000, and a loan must not exceed $50,000. Any outstanding loan balances from the previous 12 months may reduce the amount you have available to borrow. You then pay the money back into your account, plus interest, through payroll deduction.

You may have one loan outstanding at a time. The cost to initiate a loan is $50, and will be deducted directly from your plan account.

Note: If you fail to repay your loan (based on the original terms of the loan), it will be considered in "default" and treated as a distribution, making it subject to income tax and possibly to a 10% early withdrawal tax penalty. Defaulted loans may also impact your eligibility to request additional loans.

You are permitted to roll over eligible pretax and after tax contributions from another 401(k) plan, 401(a) plan, 403(b) plan or a governmental 457(b) retirement plan account or eligible pretax contributions from conduit individual retirement accounts (IRAs). You should consult your tax adviser and carefully consider the impact of making a rollover contribution to your employer's plan because it could affect your eligibility for future special tax treatments.
